New Patient Information
Your First Visit
Unlike most other practices, when you come to see us for the first time, it’s likely that the first person you’ll meet will actually be Dr. Gary Klein! He frequently hangs out in our waiting room and greets new patients. You’ll start by filling out your “paperwork” on an iPad, and then a member of our team will take you on a tour of our office. Afterward, one of our excellent hygienists will give you a dental cleaning. Then, Dr. Klein will perform a thorough oral exam. From this, he’ll come up with your customized treatment plan to help you achieve and maintain a healthy smile that will truly last.
Patient Forms
In order to help you save even more time at our office, we’ve included a link to fill out our patient forms online. Simply click the link, fill them out, and they will be digitally transferred to us for your next visit. If you forget, don’t worry—we’ll simply let you fill out the forms on an iPad. If you have any questions or difficulties getting this to work, just give our office a call and we’ll help you!
